If you are referring yourself...

You can refer yourself through your local authority, Stoke-on-Trent City Council or Cheshire East Council. You can also refer through professional services such as the police, social services or other housing providers. 

If you are referring someone else... 

We have recently introduced a new multi-agency referral form. This ensures that we have all the information we need to provide the right service for someone in need.
